PYP Authorisation Forms

If you would like to add people authorised to pick up your child from school, you can use the forms below.

Please note that authorisation for third parties other than parents (e.g. relatives, babysitter, driver, older siblings aged 14 and above, etc.) must be renewed every year.
Any additional authorized persons submitted through the 25/26 enrolment form are, of course, valid.
Should you wish to authorise another parent from the school, please send an email to office.ve@h-is.com

Below you will find the two forms: one for third parties and one specifically for an older sibling (minimum age 14):
